Google Forms is a free online tool included in your UGA Google Workspace account that allows you to create surveys, quizzes, and forms, with responses automatically stored in a spreadsheet.
Who can access Google Forms?
All UGA students, faculty, staff, and retirees with an active UGA MyID can access Google Forms through their UGA Google Workspace account.
Quick Access
Open up your web browser and go to:
https://docs.google.com/forms.
How do I access Google Forms?

Open your web browser and go to: https://docs.google.com/forms

Enter your UGA email address (MyID@uga.edu) and click Next

You will be redirected to the UGA Single Sign-On (SSO) login page:
- Enter your MyID and password
- Click Log In
- After signing in, the Google Forms homepage will load, where you can begin creating forms.
