This request form can be used to purchase Microsoft add-on licenses.
This process applies only to Microsoft add-on licenses purchased under UGA's primary Microsoft enrollment managed by EITS.
Campus IT Directors or their designated representatives should submit requests using this form to request Microsoft add-on licenses. IT Contracts and Licensing (ITCLA) will coordinate directly with the University System of Georgia (USG) to obtain quotes and process orders. Once the licenses are provisioned by USG, ITCLA will coordinate with our internal team to assign the licenses.
