MyID Account Request

These are the requirements for using the form:

  • Managers, HR reps, or sponsors should be filling out this data.
  • Passwords and other confidential information should never be sent to anyone.
  • A UGA ID (81x) must be created before submitting the ID, not required for Departmental ID request.

For Departmental IDs please use this form for new requests or for re-enabling of existing ID.
