Mass Changes (20+ users) to HR Department or Position

The USG IT Handbook (section 3.1.1.1) reads:  “System owners will update information system access no more than five business days after terminations and no more than 30 days after other personnel status changes.”
•    For individual changes, Access Request Forms should be submitted to fulfil this requirement.
•    For mass changes (20+ users), as long as users will retain the same system permissions, this form can be submitted to reduce the burden of submitting Access Request Forms for each user individually.
