PCard New Card Request

The PCard New Card Request Form is to submit for a new or current employee to become a PCard holder.

ATTENTION: Action required prior to submitting this form-- Background Check and Cardholder Training/ Documents (see below).

Background Check Prior to submitting this form, the cardholder must complete a background check through Univeristy HR. Once completed, you will receive confirmation and email from employe@uga.edu. This email must be attached to this form as part of the submission process. Please wait to receive this email, prior to submitting this form.

NEW Employee's or Positions.
  • Initiated via classifying the position as having PCard Responsibilities in UGAjobs.
    • The individual will receive an email from Accurate Background at customer_service@accurate.com
    • Follow instructions from vendor's email to create a new account or log into your existing account.
    • Sign BOTH consent forms in the Accurate Background portal. There are two separate request forms presented in the Accurate Background portal once logged in. Botha are required for PCard approvals.
      • Criminal Background package
      • Financial Background package
    • University Human Resources will update the department, cardholder, and Procurement once the background check is complete.
    • The approval email is sent employe@uga.edu.This email is what will be attached to this form upon submission.

Current Employee's

  • Initiated via an Evaluate Position Request in UGAjobs
    • The individual/department will contact their supervisor and/or HR Liaison to update their position in UGAJobs.
    • The department will add Purchase Card responsibilities to the position through an Evaluate Position Request in UGAJobs.
    • The individual will receive an email from Accurate Background at customer_service@accurate.com
    • Follow instructions from vendor's email to create a new account or log into your existing account.
    • Sign BOTH consent forms in the Accurate Background portal. There are two separate request forms presented in the Accurate Background portal once logged in. Botha are required for PCard approvals.
      • Criminal Background package
      • Financial Background package

  • The vendor, Accurate Background, may directly contact you requesting further information to process the background investigation.Please continue to monitor your email after the original consent forms are signed.

Cardholder Required as apart of submitting this form

For a new Cardholder

Approver 1 (Must be in the Cardholder’s supervisory chain unless an exception is requested)  or

Approver 2 (Cannot be a subordinate to the Cardholder)

For a NEW Approver 1 or 2 (an employee that has never filled this role)

Reconciler (Optional role) (cannot be a subordinate to the Cardholder)

For a NEW Reconciler (an employee that has never filled this role)
