PCard Change Request

The PCard Change Request Form addresses permanent changes to:

  • Changes to existing PCard roles or card attributes (to request a NEW PCard, please use the New Card Request Form.)
  • PCard single transaction limit and/or monthly credit limit
  • PCard cancellations
  • PCard related role assignment changes
  • Other PCard updates such as changes to default speed types, Cardholder name changes, temporary role changes, and etc.
  • Temporary changes to a PCard’s single transaction limit and/or monthly credit limit are handled by the PCard Temporary Change to Spending Limits Form https://busfin.uga.edu/procurement/pdf/temporary_change_to_spending_limits.pdf ).

All PCard Change Request Forms must be approved by the Approver 1.


Documentation/ Action associated with this form:

The following could be required to be filled out, based on what role is being modified and the history of the employee receiving the role.

 

Approver 1 (Must be in the Cardholder’s supervisory chain unless an exception is requested)  or

Approver 2 (Cannot be a subordinate to the Cardholder)

For a NEW Approver 1 or 2 (an employee that has never filled this role)

Reconciler (Optional role) (cannot be a subordinate to the Cardholder)

For a NEW Reconciler (an employee that has never filled this role)
