Overview
UGA affiliates are individuals who are neither employees nor students but have a university business need for credentials to access specific systems, resources, or facilities. Individuals who need affiliate access should complete this form to initiate the access request. The form contains sensitive data and should therefore be completed by the prospective affiliate.

Enter the appropriate information into the Affiliate Registrant Information fields.

From the Are you a citizen or permanent residence of the United States with a U.S. Social Security number? dropdown menu, chose the appropriate Yes or No response then enter the appropriate information into the Social Security and Date of Birth fields.

|
From the Have you previously been a UGA employee, student of affiliate? dropdown menu, chose the appropriate Yes or No response.
If applicable, enter your 81 number into the optional Previous UGA 81 number field. If you do not know your 81 number, leave that field blank.

Enter the appropriate information into the Address fields.

From the Have you EVER been a member of, involved in, or in any way associated with any organization, association, fund, foundation, party, club, society, or similar group in the United States or in any other location in the world?" dropdown menu, chose the appropriate Yes or No response.

From the Do you know which UGA faculty or staff member will sponsor your affiliation? drop down menu, Choose the appropriate Yes or No response.

Enter the appropriate Sponsor name or email in the search field, then click the Search button

From the searched results, click the Sponsor's name.
|
|
|
|
|
To accept the registration requirements, click the Check box.
|
|
|
|
Click the Submit Registration button.
